We are so excited you want to join us on our journey to leave a legacy of smiles!

Everyone will need to attach a picture of their passport. If you are a dentist, you will also need to attach copies of your Dental License, resume or cv, and your Dental School Diploma. Please have those ready before you begin so you do not lose your progress.

We also highly suggest that you have looked through the volunteer guidebook pages before you apply.

​

Each person attending must fill out a their own application.
